My scariest name goof was being passed a slip to sign for bank withdrawal at 16 and looking at the blank box and going totally blank..... Absolutely nothing, scanned women's names in head, then caught sight of name on my bank book. Was convinced I was going doolally for a month. Exam stress for you!

Yup and she was a mother of a girl in my year at school so I got totally paranoid (especially at 16). Thankfully moved house within weeks.
